Confidentiality Agreement for Healthcare Professionals with Mendez for Clinical Interventions Inc and TheAppointments.ca.

Confidentiality is a cornerstone of healthcare and is embedded in its core values. Individuals share sensitive, personal information with healthcare professionals, and maintaining confidentiality is crucial for building trust in the therapeutic relationship. It is also a key legal requirement for all regulated health professionals.

The Health Information Protection Act (HIPA) establishes the rules for confidentiality and privacy of personal health information in Canada. HIPA mandates that individual health information be kept confidential and secure. It is the fundamental responsibility of healthcare professionals to maintain client confidentiality at all times. In compliance with HIPA, healthcare professionals must ensure that the professional relationship with the client and the client’s personal information are kept confidential, within legal limitations.

Healthcare professionals must explain the principle of client confidentiality and its legal limits to clients. They are also responsible for securing client information to prevent unauthorized access to records. A healthcare professional does not collect or use information about a client without the informed consent of the client or the client’s authorized representative, nor disclose information about a client without the written informed consent of the client or the client’s authorized representative, except where disclosure is permitted or required by law.

As a healthcare professional, I understand that these rules apply both during and after my collaboration with Mendez for Clinical Interventions Inc., TheAppointments.ca. Any infringement of these rules by me may be grounds for the termination of my service from the portal without notice and may lead to legal action.

I understand that photos and videos (individual and group) will be taken and may be used for promotional materials, including newsletters, websites, presentations, and social media, to highlight and promote the organization. I am aware that these images may be edited, copied, exhibited, or published, and I waive the right to inspect or approve the finished product. This release is valid indefinitely and has no geographic limitations on where these materials may be distributed. I grant consent for my photos/videos to be used as described above.

Service Coordination Agreement with Mendez for Clinical Interventions Inc. & TheAppointments.ca

The healthcare professional agrees not to schedule follow-up appointments directly with the client. All follow-up appointments must be made through TheAppointments.ca portal to ensure proper service coordination and management. Non-compliance will result in a 20% commission charge for each directly scheduled appointment and may lead to termination of services on the portal without notice.

The healthcare professional agrees to a 25% commission for the first appointment and a 12% commission for follow-up appointments with the client.

For inclusion in the (optional) featured consultant listing on the website, the healthcare professional agrees to an additional 20% commission for the first appointment and an additional 8% commission for follow-up appointments with the client.

These commission rates apply for one year from the signing date and may be subject to change upon mutual agreement. The healthcare professional may withdraw from the service with a 15-day notice.

The healthcare professional authorizes Mendez for Clinical Interventions Inc. and TheAppointments.ca to bill clients on their behalf for services provided via TheAppointments.ca. This includes managing communications and engaging agreements with insurance companies and other relevant agencies as necessary.
